How do I prepare for my first visit?

Arrive ten minutes early so the front desk can verify your insurance and intake forms. Please bring a photo ID, your insurance card, a list of current medications (including dose and frequency), and any imaging or labs from outside providers in the past twelve months.

We are in-network with Aetna, Blue Shield, Cigna, Kaiser PPO, and Medicare. For HMO plans we'll need a referral on file before your first visit — your primary care office can fax it to (415) 555-0190. Self-pay options and a sliding-scale fee schedule are available; ask the billing team for details.

The patient portal handles bookings 24/7 — choose your provider, location, and a window that works. Rescheduling and cancellations are free up to twenty-four hours before the appointment; later changes incur a $35 hold fee that can be applied to a future visit.

Yes — most follow-up visits, lab reviews, prescription renewals, and behavioral health appointments are available by video. New-patient visits and any consult requiring a physical exam are in-person; the booking portal flags which is which.

Your medical record is stored in a HIPAA-audited environment with encryption at rest and in transit. Access is logged and limited to your direct care team; you can download a full copy any time from the portal, and we never share data with third parties for marketing.

Yes. Non-urgent questions go through the portal's secure messaging — your care team replies within one business day. Urgent symptoms after hours should go to the on-call line at (415) 555-0119; for emergencies, call 911 or go to the nearest ER.

Request refills directly from the portal or by calling your pharmacy — they'll fax us, and we approve same-day for established medications. Controlled substances may require a quick check-in visit; the portal will let you know if that's the case before you submit.
